Planes crashing. Space missions lost. Medical instruments that kill, maim and wound. Our systems are so fragile! We developers seem unable to learn from these disasters. This talk, more storytelling than technical presentation, will look at a dozen disasters and find common threads, things we all should be thinking about when building our systems. Don't plan on learning a methodology or new ways to twiddle bits! Instead we'll be using the oldest form of teaching, telling tales, to give a sense of how things fail.
