An Analysis of Causation in Aerospace Accidents
Abstract:
The losses of several aerospace accidents are described, including the SOlar Heliospheric Observatory, the Ariane 5 launch vehicle and the Titan/Milstar spacecraft, as well as the systemic factors that led to their losses. The factors in these accidents are similar to common factors found in many recent software-related aerospace losses. The similarities and parallels should help in focusing efforts to prevent future accidents.
